Forms & Policies-Pre-College - Carnegie Mellon University

Forms & Policies

As an educational program for high school students, we have many rules and regulations that students must abide by while on campus. Everything from curfew to absence policies can be found in our handbook. Those who do not comply with the rules, which are in place for your safety, are asked to leave the program.  All of the forms that are associated with the Pre-college program, including the general release and absence from campus forms, can be found in the forms packet.

Students under the age of 18 require a release form to participate in a number of activities available throughout the summer. In order to minimize paperwork, we have developed a general release for most circumstances typical to the Pre-College experience. In rare instances, parents or guardians might need to give special permission for their students to participate in exceptional activities occurring on-campus, although none of these will be required.

Curfew and the Release from Campus Form

Evening curfew is in effect for all Pre-College residential students for the entire program. Curfew begins at 11pm Sunday through Thursday evenings and 12midnight Friday and Saturday. Every student must sign themselves in for curfew each night. Students may obtain permission from their parent/guardian to be away from campus and in the custody of an identified adult by completing an Absence from Campus Form found in the forms booklet and will provide a link coming soon. This form must be completed by the parent or guardian for each absence. We do not accept completed forms via email unless they are a scanned pdf of an original form including the signature of the parent/guardian. The preferred method for receiving this form is via fax to the student Development office, 412-268-4999. No cover sheet is required when faxing the form. Questions regarding the release from campus form can be directed to pc-life@andrew.cmu.edu.
