Gradate students are required to provide medical history information and immunization records to the university. Certain immunizations and PPD skin testing for tuberculosis are required. If you do not fulfill the immunization requirements, your registration will be held and a fine will be imposed. Your department will send medical forms that you should send in advance of your arrival or bring with you. Student Health Services will provide immunizations during Graduate Orientation.
