Vision Insurance

Vision insurance is optional. Students can enroll in the Vision insurance plan during the Fall or Spring open enrollment periods only. Note than the Vision plan ends each year on July 31, regardless of when you actually enrolled into the plan.

Carnegie Mellon offers Vision insurance through The Guardian Life Insurance Company of America.

  • Plan: VSP
    Note that you WILL NOT receive vision cards this year. You need only to go to a provider in the network, identify the plan as VSP, and tell them the ID# you specified on your enrollment form (usually your SS#). Note that you do not need to know the plan number now, just your ID# and identify the plan as VSP.

Complete this form and return it to Student Health. We then charge your student account for the insurance premium fee.

For those enrolling partners as part of a Domestic Partnership, you must complete two additional forms: the Carnegie Mellon Domestic Partnership form and the Guardian Life Insurance Company's Declaration of Domestic Partnership form. When you return your completed enrollment form to Student Health, include copies of these two forms.

Cancellations/Refunds
Although students may cancel their enrollment in the vision plan at any time, there are no refunds of premiums given. Contact Student Health if you wish to cancel.
