Enrollment Forms

If you are graduating in the month of August 2008 and had enrolled in last year's Carnegie Mellon medical insurance, you are eligible to enroll in one month of this year's plan to cover you for the month of August. Complete the August graduation enrollment form listed below.

If you are enrolling for the entire year of August 1, 2008 through July 31, 2009, complete the full year enrollment form for Aug 1 2008 - July 31, 2009 found below.

If you arrived on campus later in the year, complete the prorated enrollment form listed below for the month of your arrival.

If you are already enrolled, and wish to change or cancel your insurance, refer to the Medical Insurance Change of Enrollment procedure or the Medical Insurance Cancellation of Enrollment procedure.

If instead you had previously waived and have now lost your earlier insurance, complete the prorated enrollment form for the month you lost your insurance. You'll also need to include a letter with your enrollment form indicating why you lost the prior insurance.

NOTE: We cannot let you enroll in Carnegie Mellon's contracted insurance if you misrepresented information on your prior waiver and now have claims that are not covered.
