Dental Insurance

Dental insurance is optional. Students can enroll in the dental insurance plan during the Fall or Spring open enrollment periods only. Note than the Dental plan ends each year on July 31, regardless of when you actually enrolled into the plan.

• Cancellations/Refunds: Although students can cancel their enrollment in the dental plan at any time, there are no refunds of premiums given. Contact Student Health if you wish to cancel.

CMU offers the following Dental insurance plan through The Guardian Life Insurance Company of America.

  Note that you WILL NOT receive dental cards. You need only to go to a provider in the network, identify the plan as Guardian DentalGuard plan # 394791, and tell them the ID# you specified on your enrollment form (usually your SS#).
