Health History Form and Immunization Requests
You are required to complete a Health History/Immunization form. This form allows Student Health Service to monitor the immunization status of our students to ensure that all are protected against vaccine-preventable diseases. Download the form and return it to Student Health.
Note these crucial prematriculation immunization requirements:
All full time students:
- Two doses of measles, mumps and rubella vaccine (MMR)
Additional requirements if living in university housing:
- Hepatitis B vaccination series
- Meningitis vaccination
Additional requirements for international students:
- Tuberculin skin test (PPD) within one year of enrollment.
If you are unable to obtain these vaccinations prior to arrival on campus, you can receive them from the Student Health Service prior to the first day of classes.
Failure to comply will result in administrative -suspension and removal from university housing.
