Immunization and Health Insurance Requirements

The university requires all full-time students to carry health insurance and be vaccinated for certain contagious diseases. In order to ensure coverage, all students are charged for the standard university student health insurance plan on the fall invoice. The cost of the Standard Plan is $820. It covers the student from August 1, 2007 through July 31, 2008. Students can enroll in an enhanced benefit program if they wish by filling out an enrollment form and submitting it to Student Health Services by the end of the open enrollment period in September.

If students are already insured under their parent's plan, they can file a waiver form and request to opt out of the university plan. In order to meet waiver requirements, the outside plan must be an employer or government-sponsored program and meet certain minimum standards for coverage. If the student's outside plan meets the minimum standards, the charge for the university insurance plan is removed from their student account. This action must be taken prior to the end of the open enrollment period on September 6. Hard copies of enrollment forms, benefits information and waivers will be mailed to students and online forms will be made available after June 1 at http://www.studentaffairs.cmu.edu/HealthServices.

The pre-matriculation immunization requirement affects all incoming full-time students. To meet the requirement, students must provide proof of immunity from measles, mumps and rubella to Student Health Services. Students must have had two doses of measles mumps and rubella vaccine. Documentation of a positive blood titer or of having had the disease also fulfills the requirement. Additionally, all students residing in university housing must have vaccinations for Hepatitis B and meningitis. All full-time international students must have a PPD skin test for tuberculosis within one year of enrollment to the university. Student Health Services will conduct vaccination clinics during the month of August so that students can meet the requirements in a timely fashion. Failure to comply with these requirements by the first day of classes will result in a hold being placed on fall registration and the assessment of a $50 fine. The hold will be removed once the requirement is met. A Health History and Immunization Record can be located on the Division of Student Affairs CD sent with the Admissions Packet.
