Cancellation/Refund

Use the Student medical Insurance Cancellation Form to cancel your student medical insurance policy for yourself and all dependents you have enrolled. You'll find the complete cancellation policy documented on the form itself.

• For the August 1, 2009 through July 31, 2010 academic year, complete the 09-10 Medical Enrollment Cancellation form (.pdf).

If you are cancelling only a portion of your dependents, use the Student Medical Insurance Enrollment Change form instead.
