Student Health Services
CMU's Student Health Insurance Policy
CMU has a Student Health Insurance policy requiring full-time students to carry adequate medical insurance. The official policy statement can be found here.
Each year, full-time students' accounts are charged for CMU's lowest-premium medical insurance plan. If a student has adequate alternative insurance, he or she needs to complete a waiver form so that the charge can be removed.

If any student, full or part-time, wants to enroll or upgrade to one of the higher premium medical plans, he or she should review the medical plan information, complete the enrollment form found there, and return it to Student Health.
Students are expected to make their medical insurance or waiver decisions by the insurance deadline associated with their first semester on campus each academic year. There are specific insurance deadlines for the Fall, Spring and Summer semesters. Again, you fill out a waiver once each academic year, and you hand it in by the insurance deadline of your first semester on campus that academic year.
If a student waives the medical insurance, and then his or her alternative insurance is terminated for non-medical reasons, the student must enroll on the contracted CMU medical insurance plan at that time. If it is terminated for other reasons, the student's situation will be reviewed to determine if the student is permitted to enroll in the contracted plan at that time.
Note: Students are responsible for providing their insurance information to providers. Waiver information is kept on file primarily as a record of waiver policy compliance.
