Waiver Forms
Please make sure to use the correct waiver form for the year you wish to waive at this time. Note that one form is for the current academic year, ending in the summer of 2008. The other is for the following academic year that won't begin until Fall 2008.
Hardcopies of the waiver form:
- For the August 1, 2007 through July 31, 2008 academic year, complete the 07-08 waiver form.
- For the August 1, 2008 through July 31, 2009 academic year, complete the 08-09 waiver form.
