Dental Insurance
Dental insurance is optional. Students can enroll in the dental insurance plan during the Fall or Spring open enrollment periods only. Note than the Dental plan ends each year on July 31, regardless of when you actually enrolled into the plan.

Enrollment form
Complete this form and return it to Student Health. We then charge your student account for the insurance premium.
For those enrolling partners as part of a Domestic Partnership, you must complete two additional forms: the Carnegie Mellon Domestic Partnership form and the Guardian Life Insurance Company's Declaration of Domestic Partnership form. When you return your completed enrollment form to Student Health, include copies of these two forms.
Premium and deductible costs:
- Individual: $204.56
- Family: $534.44
- There is an in-net $100 individual deductible for non-preventative services. For a family, there are up to three individual deductibles of $100 for the non-preventative services.

Cancellations/Refunds
Although students can cancel their enrollment in the dental plan at any time, there are no refunds of premiums given. Contact Student Health if you wish to cancel.
