Email Consent Form
Email transmission of information has a number of risks. Student Health Services therefore asks its patients to sign an email consent form before the staff can communicate with that patient via email.
This consent form summarizes the risks associated with email communications, and the conditions under which Student Health Services staff will communicate with students via email.
